Dr. Erickson...

has lived in the valley since 2000 with his wife, Heather. They started Capstone Family Medicine LLC and Eye Clinic LLC in 2003. Since that time, they have traveled to Sri Lanka for tsunami relief and have become a huge part of the community. Each year thereafter they added a child to their family for a grand total of four active boys!

Dr. Erickson was born and raised in Minnesota where he attended the University of Minnesota-Duluth Medical School. He attended MacNeal Family Practice Residency in suburban Chicago with emphasis in Obstetrics and Sports Medicine. He has been the team physician for the Matsu Miners since arriving in the valley and has served as chief of the medical staff at Mat-Su Regional Medical Center where he continues to remain on active status.

When he and Heather are not chasing their children, they enjoy traveling abroad, skiing, big game hunting, fishing, and beer. Dr. Erickson enjoys treating the whole family and providing the widest range of services possible to his patients.

Dr. Heather Jones...

Heather Jones is a physician in the Air National Guard and a permanent resident. She has always enjoyed hiking, cross-country skiing, camping, biking and walking or running. She also enjoys swimming and snorkeling. She is originally from California and still enjoys the sunshine so tries to escape to a sunny location at least once per year.

She completed her family medicine residency at the University of Nebraska/Offutt Air Force Base combined program in Omaha, Nebraska. Her family moved to Alaska with the United States Air Force and they have lived here for over 10 years. After deciding to leave Active Duty to allow more time to be with her family she settled in the Chugiak area where she is enjoying living. She has two teenage children who get their music abilities from their father.

was born in Chicago and raised in Wisconsin.  She graduated from the CEBU Institute of Medicine and completed her residency in family medicine at the University of Wisconsin Fox Valley.  She moved to Alaska in 2006 and worked as a contract physician at Elmendorf Air Force Base prior to joining Capstone in January 2010.

While she prefers warmer climates, Dr. Jachimiec is learning to appreciate the Alaskan outdoors – fishing, hiking, camping and kayaking in Prince William Sound.  She enjoys spending time with her husband and young daughters, cooking, traveling and cross country skiing. Dr Jachimiec has been a provider at Capstone since January 2010.

moved to Alaska in 1997 with the US Army after completing medical school at the University of South Dakota and an internship at Fort Bliss, Texas. In 2004, he completed a residency at the University of Wisconsin where he met his wife, Jessica. After a tour in Iraq, he returned to Alaska in 2005. He left the active army in 2010 and is a member of the Alaska Army National Guard.

He and Jessica live in Wasilla with their young daughters. Dr. Kelley enjoys working on their family cabin and restoring his ’55 Chevy. Dr Kelley has been a provider at Capstone since Jan 2011.

originally hails from Milwaukee, Wisconsin and graduated from the Medical College of Wisconsin. She moved to North Pole, Alaska in 2007 with the United States Air Force, in which she served 10 years. When her military commitment was complete, she and her family decided to make Alaska their permanent home.

With six children of her own, Dr. Hennemann has a particular interest in maternal/child medicine. She is a strong proponent of family and loves that her specialty truly covers “cradle to grave”. Dr Hennemann has been a provider at Capstone since November 2010.

is a recently graduated physician who is now living his dream of practicing in Alaska. He is from Idaho where he met his wife and started a family. Dr. Cook now has 4 children.

He attended medical school at Des Moines University and then completed 3 years of training at Idaho State University Family Medicine Residency. He has a strong interest in obstetrics and osteopathic medicine. He enjoys all types of outdoor activites including hunting and fishing.

She moved to Alaska in 1986 and has married and raised her family in the Valley. She has 4 sons and one daughter. Her family has flourished in Alaska. Her interests include farming and 4-H in her spare time. She also has a passion for kayaking when there is time and water.

He earned a bachelor’s degree from the Inter-Service Physician Assistant Program from the University of Nebraska Medical Center. He completed a Master’s in PA Studies Family Medicine/Urgent Care in 2001.

In his spare time he enjoys spending time with his family, all sports, hunting, fishing, camping and hiking, ATV riding, and enjoying all Alaska has to offer. Scott has been a provider at Capstone since 2013.

was born and raised in the Mat-su Valley and has been a Physician Assistant at Capstone Family Medicine and Urgent Care since 2013. She received a Bachelor of Science degree in Applied Physiology and Kinesiology from the University of Florida. Following her undergraduate education, she moved to New York City and worked at Memorial Sloan Kettering Cancer Center. She returned to Florida to attend Nova Southeastern University and graduated with a Master of Medical Science Physician Assistant degree.

She is thrilled to be a member of the Capstone community and is committed to the health of her hometown. In her free time, Megan enjoys good food, good books, traveling, and, of course, exploring Alaska in the airplane with some fishing poles.

was born and raised in North Carolina where she attended the University of North Carolina at Chapel Hill for her undergraduate degree. Prior to starting medical school, she worked as an athletic trainer and completed the prestigious Athletic Training Fellowship at the Steadman Hawkins Clinic in Vail, Colorado. She graduated from Via College of Osteopathic Medicine, Virginia Campus in 2009 and completed her residency at Nellis AFB, NV in 2012.

Her focus in residency was Women’s Health and Obstetrics. She was lucky enough to be stationed at Joint Base Elmendorf- Richardson from 2012 – 2015 which is where she and her husband fell in love with Alaska. She continued to do obstetrics and full spectrum Family Medicine until her separation from the Air Force.

Dr. Slager has been active in the Alaska Academy of Family Physicians as an ALSO instructor and with the education committee. Dr. Slager and her husband have two small children, and enjoy hiking, playing in the snow, ice fishing, photography and all that Alaska’s wide outdoors have to offer. She loves practicing full spectrum, cradle to grave Family Medicine in such a beautiful area.

had worked in the medical clinics on the Alyeska Pipeline, during it’s construction phase, and developed a desire to experience more of Alaska. He moved to the Valley in 1981, with his wife, Brenda, and their young son, Edward.

He has worked in primary care, and industrial medicine, since settling here and was a partner in a local family practice clinic for 16 years prior to accepting a position with Capstone Family Medicine in April of 2014. He is enjoying this new phase of his career. Ed was born and raised in a rural farming community, outside of Columbia, S.C.. He served as a FMF corpsman with the Marine Corps in Viet Nam, and has maintained a close relationship with local veteran organizations. He attended the University of S.C., and completed the P.A. program with the Medical University of S.C. in Charleston, S.C. in 1973.

He is a big college football fan, and has been an avid fisherman throughout his life. Enjoying their home, yard, greenhouse, and summer time on the deck gives Ed and Brenda much enjoyment. Friends and family are always welcome there.

Dr. Jeffrey Copeland...

is a Family Medicine Physician who is devoted to delivering babies and caring for families. He received his undergraduate degree in Biochemistry from the University of Florida in 1998 and his medical degree from Nova Southeastern University College of Osetopathic Medicine in Fort Lauderdale, Florida in 2002.

Dr. Copeland was trained to deliver babies and provide hospital care while serving in the United Sates Navy and is currently an active staff member at Mat-Su Regional Medical Center. He is highly skilled in many office procedures to care for as many of his patients as possible. Dr. Copeland is a thorough physician who is extremely loyal to his patients.

After serving two tours in Iraq with the United States Marine Corps and three years in Guam, he moved to Alaska with his wife of 21 years and twin boys in 2012. When he is not attending to his patients, he is usually spending time with his family doing an assortment of activities outdoors.

Daniel Schubert, MD

is a board-certified Obstetrician and Gynecologist who has been in practice for over 15 years.  He grew up in Louisiana, enjoying all that the state has to offer, but was not a fan of the heat and humidity.  There his interest in science and need to be using his hands was nurtured, channeling him towards medicine and a love of the outdoors.  In college he met his future wife and together they have enjoyed many places and experiences together, including raising a daughter.  Having previously served in the United States Army, Dr. Schubert joined the U.S. Air Force in 1995.

After graduating from the LSU School of Medicine in 1998 and completing his residency in 2002, the Air Force brought him and his family to Alaska, where they would stay for the next three years.  After his time in the Air Force, Dr. Schubert taught pelvic and reconstructive surgery and obstetrics at the University of Tennessee College of Medicine.  In 2011, he finally made Alaska his permanent home.

Away from work, Dr. Schubert enjoys experiencing the many fabulous outdoor opportunities that Alaska offers as well as other hands-on hobbies.  He can usually be found fly-fishing, hunting, camping or riding 4-wheelers with his family.  At home, he is usually tinkering with an old car or working in his woodshop where he enjoys making furniture and turning bowls.  Occasionally he will get quite enterprising and remodel a room in his home.  He especially enjoys teaching his daughter in the hands-on projects.

Dr. Schubert is certified by the American Board of Obstetrics and Gynecology and has been an ACOG member and Fellow since 2005.  Over his years in practice, Dr. Schubert has delivered over 2,000 babies and has performed countless surgeries to improve the quality of life for his patients.  He has previously served as the Medical Director for OB/GYN at Elmendorf AFB and worked with the native community across the state of Alaska.

He has joined Capstone to start Capstone Women’s Health, Obstetrics and Gynecology, and is seeing patients at the Wasilla location.  He will also see patients at the Eagle River Capstone location on an as-needed basis. Dr. Schubert performs obstetrical care and gynecological surgery at Mat-Su Regional Medical Center.  He has extensive surgical and obstetrical experience which is custom-tailored to each patient and their needs on an individual basis.  He believes that medical care should be patient and family-centered and strives to provide a comprehensive and caring plan and delivery of care.

Dr. Schubert is looking forward to caring for the women of his community, sharing in bringing new family members into the world and improving the quality of life for women of all ages, from adolescent to elder.

Dr. Kim is an active fisherman and hunter who fell in love with Alaska during his first trip to the state in 2007. He was born in Muscatine, Iowa and completed medical school at the University of Iowa, Carver School of Medicine. After completing his Internal Medicine residency at LAC-Harbor UCLA Medical Center and his Gastroenterology fellowship at the UCLA Center for Health Sciences he has been in practice in Downey California for over 25 years. He is board-certified in both Internal Medicine and Gastroenterology and is a Fellow of the American College of Gastroenterology

He started his half time practice in Kenai in January 2018 and joined Capstone Clinic, Kenai in January 2020. His style of practice is to sit down with his patients and take enough time to thoroughly address their problems.

Dr. Kim loves the people of the Kenai Peninsula and has bought a home on the Kenai River. He also plays golf, bicycles, skis, off-roads and has traveled by motorcycle throughout this great state. He has 2 grown children, who both attended Brown University in Providence, Rhode Island and live on the east coast but share their father’s love for the outdoors and traveling to Alaska.

We’re pleased to highlight Dan Costello PA-C at Capstone Clinic in Kenai.

Dan was born and raised in Denver Colorado. His family homesteaded the Territory of Colorado in 1859. He worked as a commercial fisherman in Bristol Bay Alaska for five summers to pay for his undergraduate studies at Fort Lewis College in Durango, Colorado. He then worked as an archaeologist in New Mexico, Hawaii, and Colorado. While living in New Mexico he discovered his passion for medicine working as an autopsy assistant, Emergency Room Technician, cardiac cath lab technologist, and paramedic.

Dan is a graduate of the University of New Mexico School of Medicine’s Physician Assistant (PA) Program in Albuquerque. He has worked as a PA in a variety of medical fields including Radiation/Medical Oncology, Cardiology, Geriatrics, Urgent Care and Family Medicine.

In 2006, Dan was commissioned as a Medical Officer in the Air National Guard and spent eight years serving in New Mexico and Colorado where he was also active on the Chemical Explosive and Radiation Response team.

Married with five children, Dan is active in his church, loves to venture outdoors fishing, hunting and hiking, and is a supportive part of the community. We are proud to have Dan as a member of our Capstone family on the Kenai Peninsula.

Carly Thomas was born and raised in Alaska. She has lived in Hope, McCarthy, Eagle River, Anchorage, and now resides in Palmer. She grew up with the true Alaskan upbringing of mushing dogs, hauling water, using an outhouse, fishing and gold panning since she was 5, and schooling in a one-room elementary school.  She has been a Physician Assistant at Capstone since 2017. She graduated with honors from the University of Washington and UAA in 2017. She was in the medical field for 15 years before becoming a PA-C in the capacity of a Registered Dental Hygienist. She loves Palmer for the small-town feel that is similar to what she grew up in. She has 3 kids and enjoys hiking, running, exploring, and travel. She enjoys meeting new people and exploring new places. She has taken her family to Russia, Sweden, Jamaica, Italy, Mexico, and many other locations.

Dr. Martin Glaves is an Osteopathic physician practicing Family Medicine with obstetrics and Osteopathic Manipulation. After completing his Bachelor’s degree at UAA, he attended medical school at Pacific Northwest University of Health Sciences and completed his residency at Alaska Family Medicine Residency in Anchorage.

Prior to medical school, Dr. Glaves was a volunteer EMT and a commercial set-net fisherman. Dr. Glaves is in the first generation of his family to receive a college education and is the 3rd of 4 children. As a youth, Dr. Glaves was an active member of the Boy Scouts of America, where he earned its highest rank of Eagle Scout and spent plenty of time exploring the trails and lakes of the Kenai Peninsula.

As is prerequisite to living in Alaska, Dr. Glaves loves the outdoors. He enjoys hiking and backpacking with family and friends, hunting, being a terrible fly fisherman, and anything that has to do with water, except for sinking. In his free time, Dr. Glaves enjoys time with his family in the outdoors, woodworking, learning new skills, and studying ancient technologies.

Andrea Lewis APRN, certified as a Family Nurse Practitioner, practicing at Capstone Family Medicine in Eagle River Alaska. She completed her bachelor’s degree in nursing at the Marquette University, Wisconsin while part of the US Navy. She returned to Alaska and joined the Air National Guard while working as an ICU nurse. Andrea’s Military career was cut short when she was diagnosed with type one diabetes. This led her to continue her education with Spring Arbor University, Michigan and join family medicine.

Born and raised in Alaska Andrea is excited to be serving the community that helped raise her. Outside of the clinic Andrea enjoys an active life style filled with family and animals to include dogs, pigs, a goat, a gecko, a turtle and chickens. Andrea enjoys working with her hands and is always doing a project with her family.
